Advance Care Planning ensures that your loved ones are clear about what personal & medical care decisions you’d like to have happen should you be unable to speak for yourself.

When my father was admitted into the hospital, he had no plans in place for his end-of-life, none of his wishes were in writing. He had a general power of attorney (me), but no living will or health care proxy.  That means, I had to make his decisions for him based on what I believed he would want.  Not only is that a lot of pressure, it forced me to be in my head, rather than spending what precious time I had left  with him.  I knew in that time, I would make it a mission to ensure as many people over the age of 18 do not make the same mistake.  

What are Advance Directives?

There are so many documents and forms, it can be confusing.  An Advance Directive is comprised of two documents.  First, a Living Will, each State uses their own forms, yet there is another option available, which is offered by Rose of Sharon Wellness.  You get to choose. The second is the Health Care Proxy.


A written document describing all of your wishes regarding medical care in circumstances where you are unable to make your own medical decisions.

Health care proxy

Document appointing someone you trust to ensure your wishes are enforced.  A Health Care Proxy isn't designated to make decisions for you, rather they will reinforce the choices you have already made using your Living Will.

The Simple Package

This one is for those who are just looking for some help completing the basic State Advance Directive Forms with no additional coaching or meetings.  This is your one and done option, including notarization of your documents.

Step 1

We will meet in-person to discuss the different documents you will want to consider completing.  I will educate you about documents you should have now, and those that may come up later in life.  

We will discuss what you can expect from the rest of our time together and I will answer questions you have.

Step 2

In our second session we will review the Mental, Spiritual and Emotional aspects of life and future planning.

Step 3

In our third session we will review the Physical, Practical and Financial aspects of life and future planning.

  • After-death care.
  • Funeral & Burial
  • Wills and Trusts
  • Your healthcare proxy or medical power of attorney
  • Vigil Planning
  • Medical decisions

Step 4

In our final one-on-one session we will put all the pieces together and complete your Advance Directive, Health Care Proxy and other forms.  As a notary, I am able to finalize your documents and make them official for you.

We will organize these documents, along with other important items into a single-location book or folder, so everything is easy to find when you need it.

Step 5

Our final session together will bring together your loved ones.  This will include your children, siblings, parents and/or chosen Health Care Proxies.  This can be done in-person and/or via Zoom.  We do this so that in 10 or 50 years, we don't have multiple people saying you told them this or that.  This important session guarantees that everyone is on the same page.  It also gives you the opportunity to express your wishes and address any questions.
